The form that follows is part of the Planning a Needs Assessment Workbook, developed for Project Success by Roger J.D. Chesswas, North Central Regional Educational Laboratory (NCREL) in November 1993. Project Success helps state/community agencies, educators, and parents in Illinois collaborate to meet the needs of children and families. This tool was designed to help these groups organize information and resources to reach that goal.
