Judy Caplan, coordinator of school and family partnerships at North Central Regional Educational Laboratory, describes the purpose and use of release forms. Excerpted from a videotaped interview with Judy Caplan (North Central Regional Educational Laboratory, 1997).

"The consensus usually obtained by one partner, in this case perhaps the school, creating a consent form, and there are many models available. The consent form should include clearly and explicitly all the areas in which consent is being asked for, and then one party should ask the family member to sign that. There should be, in a consent form, there should be the opportunity for family members to give consent to only some of the things, it should not be a blanket consent."
