"MEMORANDUM OF UNDERSTANDING

Recognizing the need to promote and ensure communication and collaboration between all agencies providing services to school-age children, the Fulton County School District wishes to establish an enduring cooperative agreement with the agencies noted herein:

Kentucky Commission for Children with Special Health Needs
Office of Vocational Rehabilitation
Department of Social Services
Department for Social Insurance
Western Kentucky Regional Mental Health-Mental Retardation Board, Inc.
Purchase District Health Department
Department for Employment Services
Administrative Office of the Court-Juvenile Services Division
Fulton Independent School District
Murray Head Start Program

These collaborating agencies shall cooperate in terms of:

• Training of relevant staff in procedures for agency collaboration.

• Sharing of relevant staff to initiate/implement student/client treatment plan.

• Sharing of information through oral presentation only as case conference meetings.

• Prohibiting further disclosure of confidential information without prior written approval from the parent/guardian and prior notification of the Cabinet for Human Resources and the Fulton County Board of Education.

• Agreeing to be held to the same confidentiality standards as the sharing agency.

• Agreeing to be subjected to appropriate legal sanctions for improper disclosure of confidential information.

• Sharing of facilities for on-site services on a case-by-case basis.

• Sharing of responsibility for a state-level evaluation program designed to assess the implementation and impact of the interagency program."
