Case Management



Pathways Home

The term case management refers to a variety of methods used to 1) identify the full range of services and supports that clients need, 2) ensure they actually receive those services, 3) monitor their effect, and 4) adjust the kind and intensity of those services to respond to the client's changing needs and circumstances. Case management information also can be used to help collaboratives learn about the problems children and families are having with current service delivery and to suggest ways in which policies and practices can be improved. Some initiatives hire new staff or assign existing staff to provide individual case management; others bring together staff from multiple agencies to form interdisciplinary teams. Members of these teams may provide direct services as well as work to create linkages between children, families, and appropriate providers.
